ONLINE PRIVACY POLICY:

THIS NOTICE DESCRIBES HOW INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED. PLEASE REVIEW CAREFULLY.

When you enter information into United Way of Allen County’s LEARN UNITED site, the information is used by agencies that help match people with volunteer opportunities in schools and other youth serving organizations. The information collected in LEARN UNITED will be used to enroll, verify, and track volunteers.

What follows is United Way of Allen County's (UWAC) online privacy policy, which governs personal information gathered on our Internet site (www.learnunited.org).

UWAC'S COMMITMENT TO ONLINE SECURITY

Physical, electronic and managerial procedures have been employed to safeguard the security and integrity of personal information. The UWAC encrypts information for safety purposed. All UWAC agents and contractors with access to personal information obtained on the LEARN UNITED web site is also bound to adhere to this policy as part of their contract with UWAC.

PERSONAL INFORMATION COLLECTED ONLINE

UWAC collects personal information online, including names, social security, postal and e-mail addresses, phone numbers, and other personal information to guarantee qualified volunteer applicants are properly matched and protect the safety of children (see background check release for more details).

UWAC does not knowingly collect or maintain any personal information from children under the age of 13. In addition, no part of our site is designed with the purpose of attracting any person under age 13.

HOW UWAC USES PERSONAL INFORMATION COLLECTED ONLINE

Unless you otherwise consent, we will use your personal information only for the purpose for which it is submitted, such as to volunteer and other information related to program participation, and we will use such information to provide operational notices, in program record-keeping and to conduct research on results. Our research may lead to the publication of aggregate demographical data but will not result in the reporting or publication of any personal information provided to us.

At certain points where personal information is collected on our site, you may be added on a mailing list to receive information about other UWAC programs and services and about ways to support UWAC. At any time you can add or remove your name from a UWAC list by unsubscribing through the sent notices.

HOW YOUR INFORMATION IN THE LEARN UNITED SITE MAY BE USED OR DISCLOSED WITHOUT YOUR CONSENT

In order to process a request, UWAC may send the request to organizations that are appropriate for processing. We will never sell or rent your personal information.

Unless restricted by other laws, the information can be used by or disclosed to the following without your specific written consent:

  • Authorized people who work in this agency/program for administrative purposes related to providing services for volunteer purposes;
  • The LEARN UNITED system administrator, UWAC, and its designees, and the LEARN UNITED site developer, Samaritan, Inc., may see the information for administrative purposes (for example, to check data errors);
  • Auditors or others who review the work of this agency or need to review the information to provide services to this agency;
  • Individuals performing academic research who have signed a research agreement with this agency or UWAC. Your name, social security number or other identifying information will not appear in any research report;
  • Where release is required by law (for example, a subpoena) or regulation or is requested by a government agency;
  • Authorized federal officials for the conduct of certain national security or certain activities associated with the protection of certain officials;
  • Law enforcement officials, but the disclosure must meet the minimum standards necessary for the immediate purpose and not disclose information about other individuals. A court order or search warrant may be required;
  • Others, to the extent that the law requires a specific use or disclosure of information;
  • Information may be released to prevent or lessen a serious and imminent threat to the health or safety of a person or the public; if the disclosure is made to a person or persons reasonably able to prevent or lessen the threat or harm, including the target of a threat; or
  • Where our records indicate a person or company may be engaged in fraudulent activity or other deceptive practices that a governmental agency should be made aware of.

HELP US KEEP YOUR PERSONAL INFORMATION ACCURATE

If your personal information changes or you would like to review the personal information we may have on file, please use the LEARN UNITED site to update the new information.

COMPUTER TRACKING AND COOKIES

Our web site is not set up to track, collect or distribute personal information not entered by visitors. Our site logs do generate certain kinds of non-identifying site usage data, such as the number of hits and visits to our site. This information is used for internal purposes by technical support staff to provide better services to the public and may also be provided to others, but again, the statistics contain no personal information and cannot be used to gather such information.

Our web site also recognizes the online site where a visitor “clicked", but we cannot identify the visitor or the visitor's address. Site information is used to confirm participation in the LEARN UNITED program, and may be used to advise participants of the numbers of visitors who come to our LEARN UNITED site from other sites. No personal information is collected.

A cookie is a small amount of data that is sent to your browser from a Web server and stored on your computer's hard drive. UWAC uses non-identifying cookies to provide easier site navigation and access to forms. You can still use the UWAC site if your browser is set to reject cookies. Our cookies do not generate personal data, do not read personal data from your machine and are never tied to anything that could be used to identify you.

YOUR RIGHTS REGARDING YOUR INFORMATION IN LEARN UNITED

You have the right to inspect and obtain a copy of your own protected personal information for as long as it is kept in the LEARN UNITED site, except for information compiled in reasonable anticipation of, or for use in, a legal proceeding.
You have the right to correct your protected personal information when the information in the record is inaccurate or incomplete.
You have a right to request that your personal information be provided to you by alternative means, (such as by mail or telephone), or at alternative locations (such as at your home or place of work). This agency will accommodate reasonable requests.
You have the right to receive a list of disclosures of protected personal information made by this agency or designee for up to three years prior to the date you request this information, except for disclosures for national security or intelligence purposes or to correctional institutions or law enforcement officials. If a law enforcement official or health oversight agency requests that we temporarily suspend giving you an accounting of disclosures made to them, the request must be time-limited and given to us in writing.

Other uses and disclosures of your information will be made only with your written consent.

EXERCISING YOUR RIGHTS REGARDING YOUR INFORMATION IN THE LEARN UNITED WEBSITE

You can exercise these rights by making a written request to this agency, or by making a written request to UWAC. The address is listed at the end of this Notice.

ENFORCEMENT OF YOUR PRIVACY RIGHTS

If you believe your privacy rights have been violated, you may send a written complaint to this agency. If your complaint is not resolved to your satisfaction, you may send your written complaint to UWAC. Addresses are listed at the end of this Notice. You will not be retaliated against for filing a complaint. This agency is required by law to maintain the privacy of your protected personal information, and to display a copy of the most recent Notice of Privacy Practice. This Agency reserves the right to change the Notice from time to time, and if it does, the change will affect all of the information in the LEARN UNITED site, not just the information entered after the change. The revised Notice will be posted in this Agency. You may request a copy of it from this Agency or designee.
